Ces souches pouvant être à l'origine d'infections symptomatiques de degrés de gravité variables, il convient d'évaluer leur importance clinique et épidémiologique. La présence persistante de souches toxinogènes et non toxinogènes de C. diphtheriae, associée à la variabilité de génotypes et de biotypes, implique la mise en oeuvre de nouvelles mesures de vaccination des populations. L'injection de rappel du vaccin reste la méthode de protection la plus efficace de la population adulte, groupe de population actuellement le plus exposé. La mise en place d'un bon système de surveillance, d'une prophylaxie antibiotique efficace et de programmes de vaccination actualisés, doit aller de pair avec la poursuite des études sérologiques qui permettent de surveiller le statut immunitaire de la population. Les communications du 6ème Congrès international du Groupe de travail européen sur la Diphtérie ont donné un aperçu des aspects microbiologiques et épidémiologiques de la diphtérie dans les nouveaux états indépendants de l'ex-Union soviétique. Elles ont aussi permis de procéder à une analyse internationale récente des avancées réalisées dans les domaines épidémiologiques, cliniques et microbiologiques associés à cette maladie. Methods: This is a transversal two-stage cluster survey. Data was collected for 676 randomly selected children aged 24 to 59 months, 692 children aged 7 to 10 years and 590 adolescents aged 14 to 15 years based on immunization cards. Results: The immunization coverage among children aged 24 to 59 months was high (close to 95%), with the exception of Hib (90%). For children aged 7 to 10 years, the immunization coverage was inadequate (≤ 82%) except for hepatitis B and BCG (>90% coverage). For adolescents aged 14 and 15, coverage rates were low (≤ 75%) and far below the target levels. Conclusion: We recommend a better monitoring of the vaccination status including a follow up and a catch-up vaccina-tion among children and teenagers. Epidemiological data indicate that the elderly are underrepresented among the ill individuals. Approximately 1,000 serum specimens collected in Finland in 2004 and 2005 from individuals born between 1909 and 2005, were analysed by haemagglutination-inhibition test for the presence of antibodies against the 2009 pandemic influenza A(H1N1) and recently circulating seasonal influenza A viruses. Ninety-six per cent of individuals born between 1909 and 1919 had antibodies against the 2009 pandemic influenza virus, while in age groups born between 1920 and 1944, the prevalence varied from 77% to 14%. Most individuals born after 1944 lacked antibodies to the pandemic virus. In sequence comparisons the haemagglutinin (HA) gene of the 2009 pandemic influenza A(H1N1) virus was closely related to that of the Spanish influenza and 1976 swine influenza viruses. Based on the three-dimensional structure of the HA molecule, the antigenic epitopes of the pandemic virus HA are more closely related to those of the Spanish influenza HA than to those of recent seasonal influenza A(H1N1) viruses. Among the elderly, cross-reactive antibodies against the 2009 pandemic influenza virus, which likely originate from infections caused by the Spanish influenza virus and its immediate descendants, may provide protective immunity against the present pandemic virus.
